Up and Down arrows will open main level menus and toggle through sub tier links. Enter and space open menus and escape closes them as well. How To File An Insurance Claim

The PA West Soccer Youth Division provides an accidental medical insurance policy on all participants.  The policy is an secondary medical policy and all charges must be submitted to your primary health care provider first.

The PA West Soccer policy is written as an excess policy.  This requires your primary health care provider process all claims first. You will receive an explanation of benefits from your primary provider to outline what your primary health care plan covers. DO NOT wait until your primary provider has processed the claims to file a claim with PA West Soccer. To file a claim please use the link below to file a claim online.  Please note, the deductible on the PA West Soccer secondary medical coverage is $100.00.

If you do not have primary health care coverage the PA West Soccer secondary medical coverage can serve as your primary coverage for injuries occurring during training and games.

The PA West Adult Division provides an accidental medical insurance policy on all participants through US Adult Soccer Association. Like the youth policy, this policy is a secondary medical policy and a claim must be filed with your primary health care provider.

However, DO NOT wait to file a claim with USASA.
